Position Title: Quality Improvement Consultant

Salary: Depends on experience

Job Type: Full Time

      Quality Improvement

• Oversee the performance of Health Center’s performance on various Quality Improvement Projects
• Provide support on HEDIS, PCMH and Meaningful Use at Health Centers
• Proactively assist in leading efforts for Population Management
• Understand, document and co-ordinate with other stake holders on the Data Quality Requirements
• Provide Analysis of Reports and based on Research provide Training
• Assisting Health Centers in creating or utilizing real time analytics for Population Health Management
• Develop Workflows and processes to improve patient care based on various reports
• Oversee project scope, goals and deliverables that support quality improvement goals in collaboration with stakeholders

EHR Optimization

• Is familiar with EHR Implementations and EHR Customizations
• Support EHR Customizations and assist with any customizations.
• Develop strategies for data migration from Paper Charts and/or Old Charts
• Develop Workflows and provide training to support Care coordination with the HIE Interfaces.
• Assist Health Centers in Public Health IT Reporting

Program Management

• Serves as the single point of contact between e2o Health and various Health Centers
• Manage other e2o Health resources
• Effectively communicate project status to team members and stakeholders in a timely and clear fashion
• Maintain project milestone schedules and deliverables

Other Qualifications:

• Master’s Degree in Public Health or relevant field
• Plus a minimum of 5 to 7 years of experience in a Healthcare Organization 
• Work Experience in Quality Improvement (HEDIS, PQRS, CQM)
